Trails Center for Children: Dylan's new Occupational Therapy (OT) Center

Today, Dylan visited Trails Center for Children in Sampaloc, Manila for his Initial Evaluation for his Occupational Therapy.

Trails is a pediatric center specializing in individualized services such as Speech Therapy, Occupational Therapy, and Reading Intervention. They also have summer classes and one-on-one SpEd tutorials.

Here's what happens during an initial evaluation.

When Dylan was last seen by his developmental pediatrician, she recommended that Dylan re-start his Occupational Therapy because he still had stereotypical behavior that manifests whenever he's nervous or excited.  It took a while because their class schedules kept changing and I didn't want to book an OT schedule and have to keep changing that too.

Anyway,  I booked Dylan's appointment via phone and got scheduled today.  I needed to bring Dylan's most recent doctor's assessment and P2750 - with PWD ID, or P3850 without PWD ID, good for 2 sessions.  At Trails, we filled out the necessary forms and submitted the requirements.  There was an additional form that we took home because I needed to check Dylan's baby records.  It's this form:

While we filled out the forms in the waiting room,  Dylan was taken to Teacher KC's own room for his individual assessment.  I wasn't apprehensive about Dylan being alone because he's mature enough at this point to handle being on his own.  Teacher KC also gave us forms for his teachers to fill out because they relate to Dylan's behavior in school.  All the information in these forms will be used by Teacher to evaluate Dylan's current state and formulate Dylan's OT plan.  The plan will be relayed thru a Parent-Teacher Conference to be scheduled at a later time.

This is the waiting room.  I took the picture from a high table with a couple of high chairs.  I prefer to sit there while waiting for Dylan :)  The comfort room is on the far right from this vantage point and the front desk is in front of these benches.

Here's an exterior shot of Trails.  It's located at Unit 202 2nd Floor, Grand Residences España 2, 958 A.H. Lacson Street, Sampaloc, Manila.  This is one of the main reasons why we chose Trails.  It's pretty accessible from where we live.

As you can see Dylan really likes going to Trails.  The Center is relaxing for him.  The staff and the teachers are very professional.
